• NDHIN Participation Agreement
• Amendment to Participation Agreement (NDHIN Direct Agreement)
• NDHIN Business Associate Agreement (Attachment B)

3. Once you have complete the Agreements, scan and email them to ndhin@nd.gov or you can also mail them to:
